Employee Notification

The Employee MPN Information form (See link at the bottom of the page) spells out the employee’s rights and obligations within the MPN, and provides each employee the information they need to properly access the network.

The California Labor Code and Code of Regulations require that this notification be provided to an employee at three distinct events.

  1. When the MPN network is initialized.
  2. When a new employee is hired.
  3. When an employee sustains a work-related injury.

The employer must also post conspicuous notice in the workplace of information about where and how to obtain medical treatment. (See link at the bottom of the page)

The network will become effective as of the day the notices are distributed to employees. It is important to keep adequate records of the initial distribution of the Employee MPN Information form and signed acknowledgements to support any future objections to an employee’s participation in the MPN.

Additionally, the Employee MPN Information form includes an opportunity to pre-designate a treating physician. It is a California Labor Code requirement that an offer to pre-designate a physician be offered as part of this process. If an employee completes this form and chooses a doctor who has treated and agrees to treat them in the event of an injury, the employee may get treatment outside the MPN network.
